STUDENT SHADOWING WAIVER FORM
Please complete this form prior to shadow day event.
The student named above has my permission to visit Salem Baptist Christian School on the referenced date. I understand that my student should arrive between 7:45 a.m. and 7:50 a.m. for registration in the school office as school starts at 8:00 A.M. Unless other arrangements have been made, I understand that my student who is shadowing in the school should be picked up at 3:10 P.M. Visiting students will “shadow” an enrolled student and attend each class with that student during the day. Students may bring a snack, but lunch will be provided by SBCS.
In the event of illness or injury, I do herby consent to whatever x-ray examination, anesthetic, medical, surgical or dental diagnosis or treatment and hospital care that are considered necessary in the best judgment of the attending physician, surgeon or dentist and performed by or under the supervision of a member of the medical staff or the hospital or facility furnishing medical or dental services.
I hold Salem Baptist Christian School, its officers, agents and employees harmless from any and all liability or claims which may arrive out of or in connection with my child’s participation in this activity.
I fully understand that my child is to abide by all rules and regulations governing conduct at school (including our dress code – see below). Any violation of our rules may result in him/her being sent home.
